This is a newer hand control that has been used by many of the lift chair manufacturers.  It is somewhat unusual, in that it has the control box built into the cord of the control, instead of the usual practice of having a control that plugs into a separate control box.  It has four buttons, and two, five-pin plugs for the motors.  If your control looks like the one pictured, and has the number HC R2B 03 on the back of the control, then this will be the correct one for your chair.
